Fall 2010 - Flutes Collection (10 weeks)
Make-up sessions are available should you have to miss a class for whatever reason.

Fall - 10 weeks of Class - 2 make ups (All Materials Included in Fee):

$175 - 1st Child
$70 - Sibling of 1st Child
Free - 2nd Sibling

Waiver
TERMS & CONDITIONS

I agree to waive Jammin Baby, LLC, its staff and teachers, Music Together LLC, Dance Theatre South, Durham Arts Council, Sertoma Arts Center, Ninth Street Dance, Academy for Performing Arts, Our Neighborhood School Pizzeria, Evergreen United Methodist Church of all claim for personal injury or property damage that may arise from use of the facilities before, during and after Music Together classes. I also accept responsibility for my safety and the safety of my children at the class location before, during and after Music Together classes. I agree to abide by the regulations listed in this brochure.

Regulations:
1. An administrative processing fee of 10% of the tuition payment is nonrefundable.  This will cover the PayPal fees that are non-refundable should you choose to register through Pay Pal. 
2. No refunds will be given after the 1st class.
3. Materials: CDs, songbook, and guidebook are nonreturnable unless unopened and suitable
for sale.
4. If you are unable to take a class for which you have paid, please call 919-338-2978. Every effort will be made to place you in an alternate class.
5. Fall Winter and Spring - up to 2 makeup classes per semester are permitted. Missed classes will not be credited or carried over to the next semester.  Summer Sessions - 1 make up class allowed.
6. Should Inclement Weather or an "Act of God" require us to cancel class we will try to make up the class, but shall not be required to do so if it will delay the following session.

Media Release
I give permission for photographs of myself and my children to be published in print as well as on- line for the exlusive use of promoting Jammin Baby, LLC.   I understand that our names will not be printed.
